Application Documents

1. Application.
2. Photocopy of police ID.
3. Family status certificate.
4. Two (2) ID-type photos.
5. Declaration of information and consent for the processing of Personal Data.
6. Solemn declaration from an immediate relative that he/she accepts the applicant's admission to the institution.
7. Photocopy of the Health Booklet.
8. Proof of valid insurance capacity.
9. Copy of the patient's tax return from the previous year.
10. Brief medical history.
11. Medical certificate that the applicant does not suffer from a contagious and psychiatric disease.
12. Certificate of full vaccination against the SARS CoV II virus as defined by the relevant legislation.
13. Immaterial prescription.
14. Blood tests:
    • Anti-HCV, HBsAg
    • General blood test,
    • Biochemical control
      • Sugar, Urea, Creatinine, Sodium, Potassium, SGOT, SGPT, γGT, Total cholesterol and fractions, Triglycerides, CPK, TSH, B12, Ferritin, Vitamin D
    • Urological Examinations:
      • General urine
      • Urine culture
15. Chest X-ray.
16. Confirmation of the planned action, from the Ministry of Health, vaccination as presented in the National Adult Vaccination Program 2022.
17. Medical certificate that the applicant does not have a fever and is not suffering from a respiratory infection (upon admission).
